Job Title: – Support Worker

Based: – In a number of Supported Living Tenancies and the community.

Responsible to: – Managers and Senior Support Workers

Job Purpose

To support service users in all areas of their lives as per their individualised support plans, and in a way that values their dignity, promotes choice, and respect, participation and independence.

As part of a team, working closely with families, friends, and advocates of the people you are supporting and other health and social care professionals to ensure people are supported to lead the lives they choose.

Working within agreed guidelines, policies and procedures and following instruction as determined by senior support workers and managers.

Main Duties and Responsibilities

 

To provide support to people on a one to one basis in their own homes and in the local community, in accordance with the agreed support plan and within an agreed plan of risk taking.

 

To support with everyday living skills, and provide support in such areas as: personal care, healthy eating, healthy lifestyle, emotional wellbeing, building relationships, employment, safety in the home, spiritual and cultural needs, finances, risk taking, shopping, cooking and cleaning

 

To follow guidelines to support people to make their own decisions, encouraging service users to take control of their life and develop independence.

 

To support service users to build social networks and community links.

 

To work flexibly to meet people’s support needs, ensuring the day to day support is provided as required. Support and clarification will be provided from senior support workers / managers where a change outside of usual day to day support may be necessary.

 

To preserve and respect the dignity and privacy of people and their families and observe confidentiality at all times.

 

To maintain, accurate, clear and concise written and numerical records, in respect of the people you support, team and the organisation.

 

To support people in taking medication in accordance with agreed policies and procedures.

 

To establish a good relationship with the Service User and to communicate and engage effectively with all the people involved in a person’s life.

 

To be involved in and prepare information for Service user review meetings.

 

You may be asked to accompany service users on day trips, outings, holidays, college courses, work placements.

 

To participate in personal development, supervision, staff meetings and training as required by the Manager.

 

To be familiar with and work within Supporting Options values, policies and procedures and guidelines. Taking all reasonable steps to manage and promote a working and living environment, which provides equality of opportunity and freedom from discrimination

 

To follow best practice guidance regarding Safeguarding, Mental Capacity Act, Care Standards Act, Health and Safety to promote independence.

 

To adhere to Code of conduct for Healthcare Support Workers and Adult Social Care Workers in England

 

To adhere to the Supporting Options confidentiality standards and the Data Protection Act 2018 (including GDPR) relating to information held manually or on computer records.

 

You may be asked to escort a service user to and from activities using your own vehicle, where this is the case you must ensure that you have business insurance to cover all business activities.

 

Such other duties as may be required, or directed by the Manager
